Your privacy matters. Please do not send diagnoses, medical records, appointment details, insurance information, Social Security numbers, payment-card details, pickup or destination addresses, mobility information, or other patient information through ordinary email or a public website form.
An HMR team member can review availability and collect the information needed to coordinate transportation using HMR’s approved process. Calling does not guarantee trip availability until HMR confirms the ride.
